A Coalition of Agencies Promoting Early Prenatal Care & Timely Childhood Immunizations

The Healthy Children Connection (HCC) is a program to provide nursing case management to at-risk pregnant women and mothers in the Coachella Valley. HCC is funded in part by a grant from The California Wellness Foundation. The program seeks to foster the obtainment of optimal health for pregnant women and children.

Healthy Children Connection provides

Educational information and referrals for prenatal care, childhood immunizations and other health care and social services
Access to community resources (Food, Transportation, etc…)
In home assessments and case management by Public Health Nurses
Assistance with applying for Medi-Cal, Healthy Families and other insurance programs
